Go to User CP


Scroll down to Settings & Options, click on Edit Avatar


Go to the Custom Avatar Section


You can either link an avatar or upload one from your files
Do not forget to save changes

Custom title, I believe. It could very well be both in your case...

Avatar has been fixed and I have also set your profile so that you can have a Customer User Title

Last I checked, custom title was only 100 posts.
